Summary Profile

Edmonton Pathways Program ends homelessness for people who suffer from complex psychiatric disabilities usually co-occurring with problematic or addiction substance use issues, together with significant medical and social problems by:

  • providing immediate access to permanent independent housing without requiring treatment or sobriety as a precondition for housing;

  • providing street level outreach community support and treatment with a philosophy that is founded in the principles of consumer choice and harm reduction;

  • providing services which are ‘Best Practices’ evidence informed and contributing to such evidence to find new and effective approaches to recovery and community integration for this marginalized population.

The Housing Support Worker reports to the Team Leader and works closely with the Housing and Finacial Specialist to assit /facilitate clients in moving-in, setting-up and/or exiting their housingas a component part the comprehensive support service.



Duties

  1. As a fully participating member of the ACT inter-disciplinary team, is aware of and understands the comprehensive treatment and rehabilitation/recovery plan for each client. Participates in daily and weekly team meetings.
  2. Builds supportive working relationships each client in order to readily assist them in moving in and setting up their new home:
    • Assisting with moving into housing including accessing furnishings and household goods.
    • Orienting the client to the new apartment and the necessary community resources like the location of the nearest grocery store, bus service, etc.
    • Assisting in connecting utilities and phones
    • Transporting the client to necessary initial appointments such as financial and/or ID services and assisting them in completing necessary forms and other requirements.
    • Facilitating exits and re-housing. Arranges for or does directly the cleaning, minor repairs, inspections, cancelations/transfers of utilities, etc.
  3. Continuously observes and monitors the client’s behaviors and well being. Generally reporting daily to the team as to the basic level of wellness but reporting any unusual observations to the Team Leader as soon as possible for direction and intervention.
  4. Develops solid working relationships with the client landlords in order to facilitate good relations between the client and the landlord and to ensure positive problem-solving/conflict resolution should problems occur. Develops working knowledge of Landlord-Tenant legislation so that the client can be informed of tenant rights as necessary.Is familiar with the Leasing agreements whether they be part of a Master Lease Agreement or individual client lease.
  5. Identifies any service gaps observed/encountered that are barriers to recovery for clients and advocates on their behalf.
  6. Enters and maintains electronic and written records as required. Ensures compliance with FOIP and Health Information Act.
  7. Participates in required Program and BMHC related training and meetings as required.
  8. Performs other duties as assigned.

Qualification Requirements

  • Health or Human Services related degree or diploma. An equivalent combination of education and directly related experience may be considered.
  • Minimum 2 years experience in delivering community based health or human services, preferably in providing outreach services in a ‘street/inner city/chronic homelessness’ environment or equivalent.
  • Experience in working with people who have problems in mental health and addictions.
  • Successful experience in teamwork.
  • Experience in community allied-service networking.
  • Knowledge and practice in integrating cultural contexts into service delivery would be an asset.
  • Communication skills/literacy: written, oral, computer.
  • Responsible, organized and efficient.
  • A continuous learner.
  • Physical capacity to perform the duties of the position.
  • Valid driver’s license and use of personal car.

Assertive Community Treatment Team

The Mental Health-Addictions Treatment Specialist reports to the Team Leader and is a participating member of the inter-disciplinary Assertive Community Treatment (ACT) team providing comprehensive wrap-around treatment, support and rehabilitation/recovery services for the clients of the program. It is a team case management model meaning that all the clients are clients of the Program team as a whole. This requires all team members are current with the all the clients, their situation and recovery program and goals. All treatment/recovery plans are developed conjointly as a team, including the client.

The Mental Health-Addictions Clinician, in addition to this team case management role, is responsible as for providing specialized and integrated mental health and addictions treatment/therapy, for individuals, groups and families as appropriate. This would include collaborating with allied community agencies and services as required to assist the clients in meeting their recovery goals. In a lead or conjoint role with other team members, the Mental Health-Addictions Clinician will participate in developing and delivering specialized and targeted program components such psycho-educational addiction information, depression management, etc.



Duties

  1. As a fully participating member of the ACT inter-disciplinary team, works with the team and the client to develop, implement and continuously evaluate a comprehensive strength-based assessment, treatment and rehabilitation/recovery plan that guides interventions and support to ensure effectiveness the client’s achievement of recovery goals.
  2. Conducts specialized addiction and mental health assessments as a component part of the comprehensive assessment and treatment planning process.
  3. Implements and/or leads in integrated mental health and/or addiction treatments/therapies and education within the framework of the client’s recovery plan.
  4. Participates in the team case management model of the ACT team. This requires developing solid working relationships with allied agencies and services. (See Role Description applicable to all ACT team members)
  5. Enters and maintains electronic and written records as required. Ensures compliance with FOIP and Health Information Act.
  6. Works collaboratively/builds a network with mental health and addictions services in the community to ensure community capacity building for and access to services required by the clients of the Program. This includes the 12 Step and other peer support programs.
  7. Participates in required Program and BMHC related training and meetings as required.
  8. May supervise/participate in student placements as required.
  9. Performs other duties as assigned.

Qualification Requirements

  • Minimum Masters Degree in Social Work , Psychology, Psycho-Social Rehabilitation or allied field and qualified to register or already registered to practice in AB. (An equivalent combination of education and experience may be considered).
  • Clinical experience in Mental Health and Addiction Services community treatment setting. Working knowledge and practice in acute psychiatry/crisis intervention would be an asset.
  • Clinical experience in working in addictions, integrated concurrent disorders treatment and program development. This includes advanced work in Motivational Interviewing.
  • Experience in providing outreach services in a ‘street/inner city/chronic homelessness’ environment or equivalent.
  • Successful experience in inter-disciplinary teamwork.
  • Experience in community allied service networking.
  • Working knowledgeof best practices in working with people with complex co-occurring serious mental illness-addiction-medical issues; of client centered, strength-based recovery treatments/rehabilitation strategies; of the Housing First/Pathway to Housing models; of Harm Reduction philosophy.
  • Knowledge and practice in integrating cultural contexts into practice and service delivery.
  • Experience in developing and delivering group recovery directed programs.
  • Communication skills/literacy: written, oral, computer.L

Reporting to the Team Leader, the Program Support Assistant, in addition to having and maintaining a base line knowledge of each client’s current situation and recovery goals, provides a variety of program support functions including setting-up ,implementing and coordinating systems for team and client activities and schedules; performing assistance tasks with or for team members to facilitate client service; preparing program reports for the Director/Team Leader based on program data analysis; providing a central communication hub for staff safety and communication; and providing reception/clerical duties that the team requires to ensure quality services including client records and IT liaison.



Duties

  1. Attends daily clinical review meetings to keep current with the status of each client in order be able to appropriately complete any assigned tasks relating to client service such as relaying information, setting up appointments, etc. Carefully makes note of any client interactions on the phone or in person and reports these to the Team Leader or the appropriate team member.
  2. Is “command central” for the ACT outreach team coordinating field connection though out the work day, trouble shooting connection issues including precipitating emergency protocols should the situation warrant.
  3. Conducts client service tasks as called up to do so such as confirming appointments, arranging meetings, purchasing or picking up goods/materials, etc. This may include assisting in client services, transporting clients to appointments, helping clients shop, etc.
  4. Working with team members, orders/purchases/prepares materials for client handouts, meetings, educational and group training sessions. This includes setting up and maintaining a system for managing an inventory of materials, books and other resources. This may include conducting internet and/or library searches, developing Power Point presentations, and other information gathering/resource acquisition activities.
  5. Maintains electronic data base/filing systems. Liaises with ACTers and the Primary Care IT support system. As required enters the data according to the standards set and generates reports for internal or external use as required. Trouble shoots data entry problems as required.
  6. At the Director’s request develops and maintains Program reports in a variety of formants e.g. excel spreadsheets, power point, etc. May collect data and analyze data for special projects as required.
  7. Is the IT liaison for troubleshooting technology equipment problems such as the computers, blackberries, multimedia equipment.
  8. Working with the Program Director, Team Leader and/or the team, develops, implements and maintains appropriate filing systems as required.
  9. Coordinates training and meeting schedules, inclusive of booking space, setting up equipment and arranging for meals/refreshments.
  10. Maintains confidential paper files and ensures compliance with FOIP and Health Information Act.
  11. Participates in required Program and BMHC related training and meetings as required.
  12. Performs other duties as assigned.

Qualification Requirements

  • Health or Human Services related degree or diploma. An equivalent combination of education and directly related experience may be considered.
  • Advanced IT/computer skills – Windows Office suite, Data base software, electronic client files, electronic communication and multimedia devices.
  • Experience in community based health or human services, preferably in providing outreach services in a ‘street/inner city/chronic homelessness’ environment or equivalent.
  • Experience in working with people who have problems in mental health and addictions.
  • Successful experience in teamwork.

SERVICE COORDINATION - Pathways to Housing ACT Team


Summary


Applicable to all ACT team members.


Edmonton Pathways Program ends homelessness for people who suffer from complex psychiatric disabilities usually co-occurring with problematic or addiction substance use issues, together with significant medical and social problems by:

  • providing immediate access to permanent independent housing without requiring treatment or sobriety as a precondition for housing;

  • providing street level outreach community support and treatment with a philosophy that is founded in the principles of consumer choice and harm reduction;

  • providing services which are ‘Best Practices’ evidence informed and contributing to such evidence to find new and effective approaches to recovery and community integration for this marginalized population.


The inter-disciplinary Assertive Community Treatment (ACT) team provides comprehensive wrap-around treatment, support and rehabilitation/recovery services for the clients of the program. The clients are clients of the Program team as a whole requiring all team members to be current with all the clients, their situation and recovery program and goals. All treatment/recovery plans will be developed conjointly as a team, including the client. However, individual team members will have a primary service coordination/case management role with specific clients depending on appropriate and relevant skill mix required by the client at a given time.


Duties


  1. Establish a dignifying and purposeful relationship with the client, contacting and getting to know the client as a unique individual and clarifying the role of the Service Coordinator and the client in the planning process.

  2. Collaborate with the client and the ACT team in the development of a comprehensive strengths-based service plan that is reviewed and modified with the client on a regular basis; ensure that the client’s self-identified goals are supported by the ACT team and other relevant services.

  3. Provide, coordinate and monitor services to clients to ensure the client is receiving individualized services in the following areas:

    • Housing (including landlord supports)

    • School, training and employment opportunities

    • Activities of daily living and health

    • Illness self-management

    • Relapse prevention

    • Money management

    • Advocacy for benefits and access to services

    • Transitions between hospital and community

  4. Assist the client in self-managing all aspects of their life, including their mental illness, substance use and other health conditions by such methods as information and education, coaching, practical problem-solving, and linkages with peer supports and consumer advocacy groups.

  5. Coordinate and monitor referrals to community services and advocate for client rights re: dignity, access, self-determination, privacy, access to information; as required, initiate and/or participate in case conferences that include the client, family, and other informal supports and service providers to ensure coordination of care/service and resolution of emergent issues.

  6. Provide support for crisis intervention and prevention by identifying crises and implementing prevention plans, responding to crises as they arise (mental health, physical health, non-violent crisis intervention, housing), report all crises to the Team Leader/ACT team using critical incident forms as required.

  7. Provide supports related to housing including completing paper work related to tenancy, assisting in packing and transporting belongings and furniture and setting up household, and coaching on the landlord relationship, cleaning and orienting to and connecting with the local community.

  8. Participate in a Monday to Saturday shift rotation, with some evening shifts and an ON CALL rotation.
